转移性脑肿瘤与高死亡率和不良预后相关。然而，预测脑转移患者手术预后的指标信息有限。本研究旨在探讨术前红细胞分布宽度（RDW）与转移性脑肿瘤手术患者死亡率之间的关联。本研究分析了1999年8月至2020年3月期间接受转移性脑肿瘤手术的282例患者。根据术前RDW截断值（<13.2和≥13.2）将患者分为两组，比较两组的手术结局。此外，我们进行了Cox回归分析，以评估术前RDW与1年死亡率和总死亡率之间的关联。两组在180天死亡率（6.2% vs. 28.7%，P<0.001）、1年死亡率（23.8% vs. 46.7%，P<0.001）和总死亡率（75.0% vs. 87.7%，P=0.012）方面存在显著差异。
在Cox回归分析中，RDW ≥ 13.2与较高的1年死亡率（校正风险比[HR]为2.14；95%置信区间[CI]为1.38–3.30；P<0.001）和总体死亡率（HR为1.44；95% CI为1.09–1.90；P=0.010）显著相关。术前RDW与转移性脑肿瘤手术中的高死亡率密切相关。
